From this page you can download the forms you'll need to complete your application.

The instructions below will walk you through the forms.

After downloading, please edit (rename) the file names to ADD your first and last name to the beginning of the current file names.

This will help us to keep your files separate from other applicant files when you send in your application.

File 1 – Complete On Screen

File 1 will be completed on screen and submitted by email to our Program Manager Dart Wooden at dart@acti.org.

 

This form asks for your Social Insurance Number. If you prefer, for greater security, you can call Dart at the cell number shown in file 1 and give him your Social Insurance Number over the phone.

 

File 1 is our primary application form in a Microsoft Word document. The document has been formatted to make it easy for you to type your answers directly into the pages. Once file 1 is complete and saved, email it to Dart at the address above.

 

File 1b is a PDF version of file 1 in case you don’t have MS Word. If you have MS Word, you can disregard file 1b. If you don’t have MS Word, download file 1b, print it and write your responses. Then scan and email the application or mail it using the address in file 3.

Files 2 and 3

While most of the application process is now digital, the government still requires original signatures on certain declarations in the application forms. Files 2 and 3 address this.

 

File 2 does NOT need to be printed or returned to us. It contains seven short paragraphs of information which will be referenced and summarized for signature in file 3. Download and read through file 2.

 

File 3 contains an information page and 2 signature pages. Download and print file 3. Follow the instructions on the information page.

 

IMPORTANT NOTE: before sealing the signature pages in an envelope for mailing, please scan or take pictures of them and email them to paul@acti.org with the subject line: signatures.

 

We understand that dealing with physical paper in the digital age and the age of Covid-19 can be a bit tedious but it’s really important that we get these signatures from you as quickly as possible. Please print, sign, email (the pics or scans) and mail these pages right away.
